First Penance & First Holy Communion Registration

To register for our First Penance and First Holy Communion program, please fill out the form below. This program is for children who have reached the age of reason, ordinarily in the second grade, and have completed at least one year of Faith Formation in either a parish-based program or Catholic school.

Family Covenant:
At your child's baptism you accepted the responsibility for training your children in the practice of the Catholic Faith. The covenant below outlines the responsibilities of parents and Church in raising the child in our Catholic Faith. By submitting this registration form you are agreeing to: 1) attend Sunday Mass each week with your children, 2) provide the opportunity for you and your children to receive the Sacrament of Penance at least once a year, 3) lead and encourage regular prayer at home, 4) assure that your children faithfully attend religious education classes or are prepared, and 5) attend all required parent meetings and workshops.

  • This registration form
  • $50 First Penance and First Holy Communion program fee. You can pay online after submitting this form by navigating to General > Pay Tuition on this Web site, or you can pay by cash/check to your parish office.

The following items are due prior to the celebration of First Penance:

  • Copy of the child's baptismal certificate if not baptized at Holy Cross, Mother of Sorrows, St. Charles or St. Lawrence.
  • Permission letter from your Pastor (only if you do not belong to one of our four parishes)
